The death of Jeffrey Epstein on August 10, 2019, remains one of the most scrutinized events in modern American history. While the Department of Justice and the New York City Medical Examiner’s office officially ruled it a suicide by hanging, the release of over 3 million pages of “Epstein Files” as of February 2026 has provided a clearer look at the staggering series of “failures” that occurred at the Metropolitan Correctional Center (MCC).

1. The Security Total Collapse
• The “Deleted” Footage: Following Epstein’s first alleged suicide attempt on July 23, 2019, the surveillance footage from outside his cell was reported as “accidentally deleted” due to a technical error. Epstein reportedly told his lawyers he had been assaulted, not that he had tried to kill himself.
• Camera Malfunctions: On the night of his death, digital video recorder (DVR) failures reportedly knocked out multiple cameras. Internal logs revealed motherboard and hard drive breakdowns on August 8 and August 10.
• The Guard Absence: Guards Tova Noel and Michael Thomas failed to conduct their 3:00 a.m. and 4:00 a.m. rounds, later admitting they fell asleep and browsed the internet before falsifying the logs.
2. Protocol Violations
• Suicide Watch Removal: Epstein was removed from suicide watch after only six days—a move many experts called “reckless” given the high-profile nature of his case.
• The Empty Cell: Epstein’s cellmate was transferred out on August 9, just one day before his death. Protocol required an immediate replacement; none was provided.
• The Ghost Call: One day before he died, Epstein was allowed an unmonitored call to “his mother”—who had been dead since 2004.
3. Forensic Anomalies
• The Hyoid Bone: The autopsy revealed three fractures in Epstein’s neck, including the hyoid bone. The Official Verdict
Despite these points, the DOJ Inspector General’s 2023 report concluded there was “no evidence of foul play,” attributing the death to a “perfect storm” of negligence and Bureau of Prisons (BOP) incompetence.
